Any individual who will be driving/transporting students must complete a request to review their motor vehicle driving record annually.  If you have a Virginia driver’s license, please complete “DMV Licenses Request Form – Virginia”.   If you have a driver’s license from another state, please complete the “UVA out of state” form.  The required sections on both forms are highlighted in yellow for your convenience.

• Virginia DMV Information Request Form
  • Complete the Subject Information section and the Information Requested (Driving Record Information) section.
• UVA Out of State License Form 
  • Complete and submit pages 3 and 4.

After you complete the form, print out a hard copy or email as a PDF to Elizabeth Jackson ( ej2h@virginia.edu ) in the administrative office suite.  Please allow several weeks processing time.
